FORMALITIES FOR OPENING OF ABOVE DEPOSIT ACCOUNT

(A)      INDIVIDUALS   

1 Prescribe Account Opening Form & Card
2 Introduction : Any person who is a A/c Holder of the Bank with satisfactory operation for last six months OR Person Known to the Bank
3 Two Passport Size Photograph
4 Proof of Identity : Copy of Documents, which can be produced in support of the proof of identity are-
a) Passport
  b) Permanent Account Number (PAN) card issued by the income   tax authorities
c) Voters identity card
d) Driving license
e) Letter from a recognized public authority or public servant verifying the identity and residence of the customer to the satisfaction of the bank
5 PAN  OR Form 60 (in case of person who does not have PAN)
6 Address Proof : Copy of Documents, which can be produced in support of the address are- 
a) Ration Card
b) Passport
c) Driving License
d) Identity Card issued by any Institution
e) Electricity/Telephone Bill showing residential address
f) Any document or communication issued by any authority of Central Government, State Government or local bodies showing residential address
